We are pleased to know that you are establishing a relationship with COSGRAM USA, and we look forward to creating a long-standing relationship with you, your practice, and your patients. Please read ALL of the following before proceeding.
As we begin the process to establish a new account, we will need to review a few things to ensure everything is in order.
• Completed COSGRAM USA New Account Application Form
• Tax Resale Exemption Form (if you have one)
Orders
Your product orders can be placed via two methods, either with your local COSGRAM USA representative, or via our website at www.cosgramusa.com. You will establish a username and password that will allow you to manage all aspects of your product orders, including placing them, tracking them, and re-ordering. Our goal is to ship orders as soon as possible, so we therefore try to ship all orders received by 2:00 pm PST on the same day.
Shipping and Handling
At COSGRAM USA we will share the shipping expense with your practice. COSGRAM USA will cover regular ground shipping costs for all orders over $150, and you will have the option to overnight the product at an additional cost.
Damage and Shortage Policy
Damage or shortage of products must be reported within 10 days of receipt. This should be communicated in writing to either customer service or your local sales representative. Do not take any deduction for shortages or losses without receiving a credit memo from COSGRAM USA, Corp.
Advertising
Photographic images and other digital resources will be made available for your use. We require that you adhere to the COSGRAM USA branding guidelines and al advertisements be pre-approved by COSGRAM USA Marketing Department.
Pricing
We discourage your sale of the products at anything other than their suggested retail prices.
